In order to schedule your free Initial Consult, you must first register with our Electronic Medical Record (EMR) and sign a few forms including:
- Patient Guide (which gives you a full description of the overall process)
- Privacy Policy
- Patient Registration Agreement
Signing these forms does not obligate you to working with me. It ensures HIPPA compliance and provides you with more information about the process.
